Survey and Choice Fields
These fields make surveys, feedback forms and product pickers easier to answer. Most are in the Survey & Quiz group of the Add Fields panel.
Image Choices
Choices shown as pictures, such as product colours or plan options. (In General Fields.)

| Choices | Each choice has a label, a value and an image. |
| Allow several choices | Turns it into a multi-select. |
| Columns | 1 to 6 images per row. |
| Image height (px) | 40 to 400. |
| Show labels | Shows the label under each image. |
Likert / Matrix
A grid of statements (rows) and a scale (columns), for example How much do you agree?

| Statements (rows) | One statement per row, e.g. The service was fast. |
| Scale (columns) | The answers, e.g. Strongly disagree to Strongly agree. |
| Several answers per row (checkbox grid) | Lets visitors tick more than one column per row. |
Net Promoter Score
The classic 0–10 How likely are you to recommend us? question. Set the Low end label and High end label (default: Not at all likely and Extremely likely). Survey results show promoters, detractors and the NPS score.
Ranking
Visitors drag choices into their order of importance. Keep only the top N limits how many are ranked (0 = all).
Like / Dislike
Two buttons, 👍 and 👎. Change the Like text and Dislike text.
Emoji Rating
A row of faces, from unhappy to happy. Choose 3 or 5 faces.
Semantic Differential
Pairs of opposite words with a scale between them, for example Cheap — Expensive. Enter one pair per line as Left|Right, and choose 3, 5, 7 or 9 Points.
Image Comparison
A before / after slider. Pick the Before image and After image from the media library and set their labels. It is a display field and stores no answer.
